San Juan Basin Health receives funding to continue services

The San Juan Basin Health Department’s (SJBHD) Prevention and Population Health Division is striving to provide prevention and wellness services for Archuleta and La Plata county residents.

One of the primary purposes of this division is to provide access, guidance and advocacy for those who face barriers to health care. This is a public health priority and one that was identified in SJBHD’s most recent public health improvement plan.

SJBHD is pleased to announce the continuation of these services during the next state fiscal year (July 1, 2015-June 30, 2016).

• SJBHD receives funding to employ a nurse to work with the Temporary Assistance for Needy Families (TANF) staff at the Department of Human Services. TANF assists parents in ensuring children are immunized, assesses the need for family planning services, ensures children in the program are assessed for developmental delays and coordinates any necessary referrals, and helps parents set health goals with a care plan. SJBHD received $86,619 for this program.
