San Juan Basin Health Department is conducting a Back-to-School Immunizations Day on Thursday, Aug. 30, from 10 a.m. to 6 p.m.
Parents need to make an appointment by calling 264-2409.
Immunizations are an important part of our children’s health care and Colorado law requires that children going to school be vaccinated to prevent vaccine preventable disease.
Please bring your child’s immunization record to your appointment. We accept Medicaid, CHP+ and Rocky Mountain HMO insurance. Your child’s insurance card is required at time of service.
San Juan Basin Health asks for a minimal fee of $14.70 per immunization to cover costs of administering the vaccination. Otherwise, the vaccine is provided free thanks to the federally funded Vaccines for Children program. If a family cannot afford to pay, we will still provide the necessary immunizations.
